A complete failure of communication coupled with poor nursing care. The staff was disorganized and failed to return phone calls. Repetitive breakdown of communication between staff members resulted in delayed medicinal treatments. Crossroads staff failed to show up when scheduled on several occasions. When my loved one passed, she had been in excruciating pain the last 24 hours because of the lack of consistent care and communication with me and the care facility. Heartbreaking!
